The year-end reporting form is for use on the last month of the year. It is to be completed and returned to the Conference Office by May 10. Use this form instead of the standard monthly report for your April month report.    Year End Report

The monthly reporting form should be completed and in the Conference Office no later than the 10th of the month following the month being reported. This form is generic- you must fill in the month you are reporting. If you use the forms found in the secretary packet there is one for each month with the month already filled in on each.    Report Form

The investiture requisition shoul;d be completed and in the Conference Office my the end of March. This form provides you a way of ordering investiture supply such as patches and pins for honors and classwork. These supplies can not be ordered directly from Advent Source but, instead, must be ordered through the Conference Office.    Order Form
